Director Nomination Form
This form is used to be nominated as a Director of the Ambulance Provident Fund for election at an Annual General Meeting. The form can only be submitted once the AGM is declared, must be signed by the applicant, nominator and second and submitted to the Returning Officer by the prescribed date.
Nominations for Director positions for the 2022 AGM will be announced around September 2022. Completed nominations are to be submitted to the following email address by the nominated date advertised at the time: AGM@ambprov.org.au
Vacant positions will be filled at the AGM.
In line with the constitution 1/3 of the available Board positions are elected each year. Members interested in applying for Director positions on the Board should complete and return the form available in this location by the date to be advertised in 2022.
Proxy Vote Form
This form is used to provide a proxy vote to a member who is attending the Annual General Meeting or any Special Meeting. The form is modified as necessary to reflect specific agenda items. The form must be presented to Returning Officer no later than 24 hours before the commencement of the Meeting. The link will be activated prior to each AGM or Special Meeting.
