2024 Director nominations - Open 01 October 2024: NOMINATIONS ARE NOW CLOSED.

This form is used to be nominated as a Director of the Ambulance Provident Fund for election at an Annual General Meeting. The form can only be submitted once the AGM is declared, must be signed by the applicant, nominator and seconder, and submitted to the Returning Officer by the prescribed date on the form.

Nominations for Director positions at the 2024 AGM are open from 01 October 2024. Completed nominations are to be submitted to the following email address by the nominated date advertised at the time on the form: AGM@ambprov.org.au

Vacant positions will be filled at the AGM.

In line with the constitution 1/3 of the available Board positions are elected each year. Members interested in applying for Director positions on the Board should complete and return the form available in this location by the date advised on the nomination form.

Note: Nominees must be registered with ASIC and be able to provide a current valid Directors ID number on the nomination form.

This form is used to provide a proxy vote to a member who is attending the Annual General Meeting or any Special Meeting. The form is modified as necessary to reflect specific agenda items. The form must be presented to the Secretary / Returning Officer before the commencement of the Meeting. The 'due by' dates will be advertised prior to the meeting. The link will be activated prior to each AGM or Special Meeting.
