BENEFICIARY CLAIM FORM:
The Beneficiary Claim Form is to be used at the time of a members death. The form will need to be completed and signed by the nominated members' beneficiary and then submitted to the APF secretary. The APF is committed where possible to making beneficiary payments within 48 hours of being notified of the members death.

ANNUAL GENERAL MEETING NOTICE, AGENDA, DETAILS & FORMS
The 2026 AGM for the Ambulance Provident Fund Ltd is scheduled for 10 November 2026 at 1900hr. The AGM will be held online only. All members will have received the AGM notice via email. A link to the Secretary email address in that email provides for you to be able to indicate if you would like to attend online (Teams) or submit an apology. A link for members to join online will be sent out when requested by those members wishing to attend.

2025 Director Nominations - Details and Nomination Form
2026 Director Nominations - Due COB Wednesday 5 August 2026
To be nominated as a Director of the Ambulance Provident Fund Limited for election at the 2026 Annual General Meeting, use the form located in the links below. The form must be signed by the applicant, nominator and seconder, both of whom must be members, and submitted to the Secretary by the prescribed date on the form. Nominees must have a Director ID prior to appointment.
Nominees must be a Full Member to nominate, have held membership for a continuous period of at least 24 months immediately prior to the closing date for nomination. There are three (3) Director positions available for three-year terms.
Nominations are open from 20 July 2026. Completed nominations are to be submitted to secretary@ambprov.org.au by COB Wednesday 5 August 2026.
Vacant positions will be filled at the 2026 AGM.

Details of the AGM, including voting instructions for director nominees, will be sent out in October. The date set down for the AGM is 10 November 2026 (TBC). Members will be able to attend online (see above).
Proxy Vote Form
Note: The APF received only two nominations for the position of Board Director for this years AGM. There are actually three vacancies, and hence, there is no need to go to an election. The nominees will be appointed by the Board at this upcoming AGM. Members will be notified of the new Board following the AGM.
This form is used to provide a proxy vote to a member who is attending the Annual General Meeting or any Special Meeting. The form is modified as necessary to reflect specific agenda items. The form must be presented to the Secretary / Returning Officer before the commencement of the Meeting. The 'due by' dates are advertised on the form for those sending via email or post.
2025 APF Director Proxy Form - Not required this AGM. Nominee numbers do not meet or exceed positions available.
