If you wish to join the Ambulance Provident Fund, you can apply by using the following online process below. Current members can also update their details either through our online Member Management System.

BENEFICIARY CLAIM FORM:

The Beneficiary Claim Form is to be used at the time of a members death. The form will need to be completed and signed by the nominated members' beneficiary and then submitted to the APF secretary. The APF is committed where possible to making beneficiary payments within 48 hours of being notified of the members death.

The 2025 AGM for the Ambulance Provident Fund Ltd will be held in November 2025 at the Branksome Hotel Mascot & available to attend online also. Actual date TBA - all members will receive the AGM notice via email.

2025 Director Nominations - Details and Nomination Form

2025 Director Nominations - Open 18 July 2025:

To be nominated as a Director of the Ambulance Provident Fund Limited for election at the 2025 Annual General Meeting, use the form located in the links below. The form must be signed by the applicant, nominator and seconder, both of whom must be members, and submitted to the Secretary by the prescribed date on the form.

Nominees must be a Full Member to nominate, have held membership for a continuous period of at least 24 months immediately prior to the closing date for nomination. There are three (3) Director positions available for three-year terms.

Nominations are open from 19 July 2025. Completed nominations are to be submitted to the following email address by the nominated closing date - Saturday 09 August 2025: AGM@ambprov.org.au

Vacant positions will be filled at the 2025 AGM.

Note: Nominees must be registered with ASIC and be able to provide a current valid Directors ID number on the nomination form.

Details of the AGM will be sent out in late September / early October. The date set down for the AGM is TBA November 2025. Members will be able to attend online.

2025 Nominees will be announced shortly after the process closing date. Additionally, voting instructions will also be provided then.

Proxy Vote Form

This form is used to provide a proxy vote to a member who is attending the Annual General Meeting or any Special Meeting. The form is modified as necessary to reflect specific agenda items. The form must be presented to the Secretary / Returning Officer before the commencement of the Meeting. The 'due by' dates are advertised on the form for those sending via email or post.
